Mandatory Forms
All players need to print the following forms and complete all info, give to Coach Mac during the first team meeting.
Do not mail these in early!
Incomplete forms or improperly completed forms could lead to ineligibility or lack of Gordon State College secondary insurance coverage.
FORMS ALL PLAYERS NEED TO COMPLETE EACH YEAR: All players, incoming and returners, need to complete the physical after the listed date. July 1st, 2019.
Physical Exam Form - this is the required physical form. Make sure your Dr is completing the physical on the required form that we have provided. It is your doctor's discretion to what all needs to be completed with your physical. All 4 pages should be completed by you and your Doctor. Physicals must be completed on this form and can not be performed before July 1st, 2019.
INSURANCE INFO AND FORMS: All incoming players will need to complete these. Returning players will only need to complete the following insurance forms if any information has changed since you completed last years forms.

NJCAA Eligibility Affidavit - with a copy of your high school diploma.

Players must have an updated Medical Evaluation form (physical) and proof of health insurance before they can participate team practice. Players will not participate in any scrimmages/games until the athletic departments has received all completed forms. NO EXCEPTIONS!
